With your time as important as it is, you might want to consider filling out a registration form before you arrive for your imaging, lab or surgery center appointment. Please print the registration form, complete it, and bring it with you on the day of your appointment. You will still need to register at the reception desk to present your insurance card and to pay your co-payment, if required.

If your form is incomplete or needs clarification, then the receptionist may ask you additional questions when you arrive for your appointment.
